See attached pages, for you to approve/sign, on each of the two marked areas, to then forward back to us. You can print off these pages, sign in the two marked areas, and mail back to us.

When I receive these signed forms back from you, I will then call you and we will complete/confirm, that all the forms are filled out correctly. You are welcome to fill in any of the missing information, if you'd like. Athene will provide you, with a copy of all the completed forms. All we are doing now is opening up an account, to lock in any bonuses/rates and to get you an account number. Once an account number has been set up, then we can take steps, with other forms, at a later date, to transfer funds, etc. Certain states require different forms or additional forms.
